Treasure Coast Surgical Center offers On-line Pre-Registration which can be completed on their website.

Treasure Coast Surgical Center

If you require a paper version of the Pre-Registration Form, please call the office at (772) 466-7200 or you may call the staff at the staff at the Treasure Coast Surgical Center directly at (772) 467-1960. Be sure to complete and fill out all pre-registration pages either online or by paper.

Please note that the Treasure Coast Surgical Center requires these forms at least two (2) business days prior to your scheduled procedure. When completing the form online, please read the instructions carefully listed on the On-line Pre-Registration page. If an email address is supplied you will receive an e-mail confirmation. To ensure your form was received by the Treasure Coast Surgical Center, please call their office at (772) 467-1960.

If you need to schedule your procedure, please call the office (772) 466-7200 before calling the Treasure Coast Surgical Center. Once your appointment is scheduled, please remember to bring both your insurance cards and photo identification upon check-in with the registration desk and your time of service, on the day of your procedure.

Release of Information

Records will be fax or shipped within 5-10 Business days. Should you need to check the status of your request, please call the office (772) 466-7200.

How much will it cost to receive my medical records?

For patients, there may be a charge of $1.00 for pages 1-25 then .25 cents for pages (26 and up) plus the cost of shipping and handling.
